New Student Registration Forms
To register a new student at Our Mother of Sorrows School, please download the form packet below.
Please print and fill out one copy for each student.

There is a one-time-only, non-refundable $50 registration fee for new applicants. However, the $50 is credited toward your first tuition payment should your child enroll and attend the school.
